    Hi from Wisconsin,

    We are curious how other CCC programs might be collecting, measuring, and reporting on the use of survivorship care plans within your coalition? And if you are measuring the use of survivorship care plans, what is the source of that data?  (survey, EHR data, etc.?)

    In the past, we've paid for Module 14: Cancer Survivorship: Course of Treatment to be added to BRFSS, and that includes the questions "Did any doctor, nurse, or other health professional ever give you a written summary of all the cancer treatments that you received?", "Have you ever received instructions from a doctor, nurse, or other health professional about where you should return or who you should see for routine cancer check-ups after completing your treatment for cancer?" and "Were these instructions written down or printed on paper for you?"

    We also, in the past (in the last 5 year grant cycle) had sub-contractors of specific cancer centers who did work around cancer survivorship care plans (for example, making their own cancer survivorship care plan templates for certain kinds of cancers) and they collected their own data about use of those plans within their own cancer center.

    Thanks, Katie. I appreciate the thoughts. We too have paid for the survivorship module in the past and won't be able to maintain the cost in our state. We've been trying to explore other areas for survivorship data so always open to other ideas if anyone has any :)

    Yeah, adding BRFSS questions is expensive!

    I forgot to mention that we've also been promoting the Cancer Support Community's Cancer Experience Registry, which I know you're aware of already - I'm not sure if that has cancer survivorship care plans questions, but we're up to 75 responses so far in Iowa, and if we get to 100 we get a special report and I hope that will be useful as well. We have a few organizations that have also gotten unique URLs for the survey as well and hope to have their own tailored reports if they get enough responses.
